Objectives of the  studies

The anti-ageing effect of pure silk (100%) tissue with ultra-fine silver (999.9) nano-coating (Mrs KUDRINA as inventor) was evaluated on healthy volunteers and with

a biological evaluation on an experimentally aged human skin model.

 

Methods

The first clinical study on 20 volunteers allowed to evaluate the anti-ageing efficacy of an eye mask after 1 night application on bags under eyes, crow's foot wrinkles, dark eye-rims and hydration.

The second study on 13 volunteers was conducted after 1 month application of a helmet mask to evaluate skin glow and skin density.

For the biological evaluation, silk tissue nano-coated with silver was applied on skin fragments experimentally aged by UV (A and B) and maintained in survival during 14 days.

 

Results

The first clinical study has shown the efficacy of 1 night application of the eye mask, with a significant decrease (23.5%) of bags under eyes (dermatological scoring), of crow's foot wrinkles by 6.6 % (profilometric evaluation by image analysis), of dark eye-rims by 3.2 % (chromometric evaluation) associated with a significant increase of hydration around eyes by 4.1% (corneometric measurement).

 

The second clinical study, after 1 month of daily use of the helmet mask, has revealed a significant increase of skin glow by 49% (dermatological scoring) and of skin density by 8.5 % (echographic measurement).

 

The application of silk tissue nano-coated with silver on human skin fragments maintained in survival allowed to obtain a significant increase by 60% of epidermis renewal (mitotic index increased) and of  glycosaminoglycans rate (histologic analysis) associated with a significant decrease of free radicals by 13% (H2O2 assay).

 

 

Conclusion

            In our dermatological experience, it is the first time we obtained a complete correlation between clinical and biological anti-aging effect of a cosmetic product:

 

-       Anti-ageing effect with crow's foot wrinkles decrease in clinical environment correlated with mitotic index increase and free radicals decrease observed in the biological skin model.

-       Hydration increase around eyes correlated with glycosaminoglycans increase in the biological skin model.

-       Firmness and cutaneous density increase correlated with collagen synthesis in the biological skin model.

Abstract

 

„Photobiomodulation“ respectively „Low-Level Light Therapy (LLLT)” is the application of light of different wavelengths, power and energy densities in various fields of medicine, e.g. for tissue regeneration and scarring, blood perfusion and pain therapy.

In this lecture, we provide a brief survey of the current state of research in the field of photobiomodulation in plastic, aesthetic and reconstructive surgery and we present preliminary results from the first controlled laboratory trial on the effect of pulsating cold red light on angiogenesis at the chick chorioallantoic membrane (CAM), which is a well-known bioreactor.

 

As light source a Repuls7 (medical product class 2b by Repuls Lichtmedizintechnik GmbH, Vienna), consisting of 7 LEDs (Light Emitting Diodes), emitting pulsating incoherent red light at a wavelength range from 600-660nm was applied. Photos from the blood vessels trough a reflected-light microscope were taken once every day and analysed for the junctions of the blood vessels. In the CAM assays treated with LLLT the number of junctions was higher compared to the CAM assays in the control group. 

 

Our results reveal that the effect of photobiomodulation at 600-660nm wavelength on angiogenesis can be verified in comparison to a control group on the CAM model. Angiogenesis is an important factor regarding wound healing and tissue regeneration. Due to the specific properties of the chicken CAM model, the information value from the clinical point of view is higher than in “conventional” pure in-vitro experiments.

The effect of Autologous Platelet Rich Plasma (PRP) and Hair Follicle Stem Cells (HFSCs) in Male Pattern Hair Loss.

Platelet-Rich Plasma (PRP) has emerged as a new treatment modality in regenerative plastic surgery, and preliminary evidence suggesting that it might have a beneficial role in hair re-growth. Here we reported the results of a randomized, trichoscan evaluator blinded, placebo half-head group study to compare the hair re-growth with PRP versus placebo. To investigate the safety and clinical efficacy of autologous PRP injections for pattern hair loss.

PRP, prepared from a small volume of blood, was injected on half of the selected patients’ scalps with pattern hair loss. The other half was treated with placebo. Three treatments were given for each patient, with intervals of 30 days. The endpoints were hair re-growth, hair dystrophy as measured by dermoscopy, burning or itching sensation, and cell proliferation as measured by Ki-67 evaluation. Patients were followed for 2 year.

23 patients were enrolled, three of whom were excluded. At the end of the 3 cycles of treatment, the patients presented clinical improvement in the mean number of hairs, with a mean increase of 33,6 hairs in the target area, and a mean increase in total hair density of 45,9 (number of hairs/cm2) compared with baseline values. No side-effects were noted during treatment. Microscopic evaluation showed the increase of epidermis thickness and of the number of hair follicles two weeks after the last PRP treatment compared to baseline value (p<0.05). We also observed previously an increase of Ki67+ keratinocytes of epidermis and of hair follicular bulge cells, and a slight increase of small blood vessels around hair follicles in the treated skin compared to baseline (p<0.05). Our data clearly highlights the positive effects of PRP injections on male pattern hair loss without major side-effects. PRP may serve as a safe and effective treatment option against hair loss, and calls for more extensive controlled studies.

Introduction

Fat tissue, due to its high concentration of pluripotent stem cells, has a role in aesthetic medicine and reconstructive surgery. However, poor survival of the transplanted cells still limits the usefulness of this material in regenerative medicine. Several studies indicated that platelet rich plasma (PRP) may improve adipose tissue viability due to its growth factor content. However, pro-inflammatory agents such as fibrin and leukocytes are also present in PRP which are not desired in these indications. The present study aimed to investigate the effects of hyperacute serum, a novel cell- and fibrin-free blood derivative on the adipogenic lineage in vitro.

Methods

Metabolic activity and proliferation rate of human mesenchymal stem cells (MSCs) cultivated in media supplemented with three types of serum additives (fetal calf serum (FCS), human PRP or hyperacute serum). Hyperacute serum was isolated from glass-activated platelet rich fibrin within 10 minutes of blood drawing to prevent the propagation of inflammatory cascades.  Cell proliferation was determined using a tetrazolium assay. Adipogenesis was evaluated in standard and pro-adipogenic media  and tested by oil red staining, triglyceride content and expression of specific genes. Adipogenic and adipokine proteins in the sera were measured by multiplex ELISA assays.

Results

Proliferation of MSCs was supported by both FCS and hyperacute serum in a time-dependent manner, but surprisingly, PRP had a much weaker effect (change in proliferation rate after 5 days relative to metabolic activity on day 0 - FCS : 5.4 fold, hyperacute serum : 5.8, serum free 1.9, PRP 3.0 p<0.05). Lipogenesis was only observed in groups with adipogenic differentiation medium, with hyperacute serum showing a significantly stronger effect than PRP. This was confirmed by intensive accumulation of lysochrome dye in lipid droplets, higher triglyceride concentration and elevated expression of specific adipogenic genes. Interestingly it was also observed that in hyperacute serum group the production of COL1 was highly upregulated. Measurement of lipogenic proteins in the sera revealed,  that both PRP and hyperacute serum are abundant in them, PRP also contains anti-adipogenic factors which explains its weaker and less reliable effect.

Conclusion

Hyperacute serum provides more robust support than PRP in MSC proliferation as well as lipogenic differentiation, indicating that it may be a better adjuvant in fat grafting procedures.
